1. As of January 1, 2023, while we may still receive donations via CashApp or PayPal, we no longer prepare contribution / giving statements from those payment platforms. Please view your CashApp or PayPal account statements in your profile for your yearly contributions directly from those platforms.

2. We encourage you to use our online giving platform which allows you to receive your statement early, quarterly throughout the year and promptly at the end of the year, visit www.lhmiinc.org/donate for more to sign-up.

3. As of January 1, 2022, we were transitioning from printed statements and after March 1st, 2023, ALL contribution statements moving forward will be delivered electronically to your most current email contact on file.

• To better serve you, by delivering your contribution/giving statements each year to your most current contact, we encourage you to use the form on this page to share your most current e-mail address and contact information for the preparation of your contribution/giving statements.
